How do I Create Client Forms - Uploadable & Configurable

Steps to create - Uploadable Client forms



1) Go to the setting option then click on Manage Questionnaire.

2) Click on create new survey.

3) Mention the survey name & Description.

4) Select Form Type as Upload Form in drop down.

5) Select Status as Active.

6) Mention the Printable Content in Printable Content box.

7) If you want to add client form with any specific follow up type then check the Yes radio button.

8) If you don't want to add client form with any follow up type then check the No radio button.

9) Click on Create.

 

Steps to create - Configurable forms:


10) Then create a new section for the survey by clicking on Create a new section.

11) Mention the title in title box.

12) Select Status as Active.

13) Then create a question for the section by clicking on Create a new question.

14) Mention the text in Question Text box.

15) Select Status as Active.

16) Select the question type in drop down as required.

17) Select the Compulsory option in drop down as required.

18) Click on Create.

19) Then create a response by clicking on Create a new response

20) Mention the text in Response Text box

21) Select Status as Active.

22) Click on Create.
